An application and a declaration (according to established forms) are submitted for the assignment of a housing subsidy.
Depending on the circumstances, other documents may be provided (if available):
housing rental agreement - if the applicant lives in a rental housing;
a copy of the debt restructuring agreement - provided there is debt and the agreement is concluded.
If necessary, citizens can provide other documents that are important for consideration of the issue on the merits, for example: a document confirming the fact that a person does not live at the registered/declared place of residence, in particular, certificates, statements about residence without a residence permit, including a certificate internally displaced person.
A person applying for a subsidy shall present his/her passport of a citizen of Ukraine or a temporary certificate of a citizen of Ukraine (for foreigners and stateless persons – a foreigner's passport document or a document certifying a stateless person or a corresponding card).
If there is no document on the child's vaccination, you can contact the family doctor for a certificate.
Information about vaccinations is recorded in the card of preventive vaccinations. Without it, the child does not have the right to attend children's groups (kindergartens and schools).
The family doctor creates a vaccination certificate from the relevant records in the outpatient card (form 025/o) or the Electronic Health Care System (EHS). If vaccination data is not available in the EOHS, the doctor must duplicate the records from the medical record in it.
However, in the absence of documentary confirmation of vaccinations, it is considered that the person has not been vaccinated. In this case, it is necessary to do all mandatory vaccinations again.
For citizens who have reached the age of 14 and are receiving a passport of a citizen of Ukraine for the first time - the administrative service of issuing a passport of a citizen of Ukraine in the form of a card is provided free of charge.
