Form 6 of the Armed Forces is a certificate that will help to obtain the status of a participant in hostilities. Its sample can be found in Appendix 6 to the Procedure (as amended by Resolution No. 887 of the Cabinet of Ministers of Ukraine dated August 22, 2023).
The certificate of participation in combat operations and the performance of tasks (Appendix 6) is issued by the commander of the military unit in which the soldier served directly at the request of the soldier.
During the period of martial law on the territory of Ukraine, the procedure for conducting a medical and social examination is simplified.
If a person needs to be diagnosed with a disability, but he cannot come for an examination, the MSEK can make such a decision in absentia on the basis of a referral from the medical advisory committee of the medical institution.
If under some circumstances you or your child cannot pass the examination for the examination of the LCC and referral to the MSEK, the period of re-examination is considered to be extended for the entire duration of martial law and for 6 months after its termination/cancellation.
The commission must make a decision within five working days from the date of receipt of documents from the medical advisory commission.
