Parents, relatives must send an application and copies (scans or photos) of the necessary documents to the email address birth@cv.minjust.gov.ua.

After birth registration, a copy of the child's birth certificate is sent to applicants' e-mail address.

To obtain the original birth certificate and, in the cases provided for by law, an extract from the State Register of Civil Status Acts of Citizens about the state birth registration of the child's parents, relatives (grandmother, grandfather, adult brother and sister, aunt, uncle, stepmother, stepfather of the child) or their power of attorney representatives must apply with the originals of documents to any department of state registration of civil status acts (RACS) on the territory controlled by Ukraine.

List of documents to be submitted to register the birth of a child:

1. Application for birth registration (optional form), which states:

child (surname, first name and (if available) patronymic, gender, date of birth, place of birth); mother and father (surname, first name, patronymic, date of birth, citizenship, registered place of residence (if available), passport details). In the case of the birth of a child to a mother who is not married, in the absence of a joint statement of parents/father, only the name and patronymic of the father are indicated in the statement;

of the applicant (surname, first name and patronymic, passport details) in case the application for birth registration is not submitted by the child's parents, but by another person.

1. Passports of the parents (one of them) and the applicant (if the state birth registration is not carried out by the parents, but by another person).

2. Marriage certificate (if available).

3. Medical document that is the basis for birth registration:

4. a medical certificate of birth in the form 103/o, filled out by a medical worker who took delivery, carried out the first examination of the child or mother after delivery, including outside a health care facility; a certificate (in any form without a stamp, with information about the child) issued by a medical professional who was present during the birth or who conducted the first examination of the newborn.

You can also submit an application to establish the fact of the birth of a child through the State System "Electronic Court"

The following must be added to the application:

1. copies of documents confirming the birth of the child (medical birth certificate, other medical certificates);

2. copies of passports and identification codes of parents / one of the parents;

3. a copy of the marriage certificate (if available);

· other evidence of the birth of a child (if available).

According to Order No. 76 during martial law, conscripted employees working for :

1. enterprises, institutions and organizations that have been assigned mobilization tasks (orders);

2. enterprises, institutions and organizations that produce goods, perform work and provide services necessary to meet the needs of the Armed Forces and other military formations;

3. enterprises, institutions and organizations that are critically important for the functioning of the economy and ensuring the livelihood of the population in a special period.

To obtain the status of a critically important enterprise, in particular, in the field of agriculture, it is necessary to meet three or more criteria approved by Resolution No. 76:

1) the total amount of taxes, fees, payments accrued, withheld and paid to the state and local budgets, in addition to customs payments, during the reporting tax year exceeds the equivalent of 1.5 million euros;

2) the amount of receipts in foreign currency, except for credits and loans, for the reporting tax year exceeds the equivalent of 32 million euros;

3) the enterprise is of strategic importance for the economy and security of the state in accordance with the list of state-owned objects of strategic importance for the economy and security of the state;

4) the enterprise is important for the branch of the national economy or meeting the needs of the territorial community;

5) absence of arrears for the payment of a single contribution to mandatory state social insurance;

6) the average salary of insured employees at the enterprise for the last calendar quarter is not less than the average salary in the region for the IV quarter of 2021;

7) the enterprise is a resident of Diya City.

Next, the enterprise, institution or organization applies to the Ministry of Agrarian Policy to obtain the status of critical importance in the field of agriculture.
