A person who has been discharged from military service must apply to the TCC at the place of registration with the following documents:

  • statement regarding the consideration of the issue of obtaining UBD;

  • consent to the collection and processing of personal data;

  • certified copies of pages of the passport of a citizen of Ukraine;

  • a document on the assignment of an identification number;

  • two 3×4 color matte photos.

The TCC requests the military unit to send documents to the TCC, namely:

  1. a certificate of direct participation in measures to ensure defense, the form of which is approved by the new Appendix 6 to Resolution of the CMU No. 413;

  2. at least one of the following documents: extract (copy) of combat reports; extract (copy) of combat logs (operational tasks, management of operational situation); extract (copy) of logbooks, flight sheets; extract (copy) of materials of special (official) investigations into the facts of injuries, contusions, mutilations, and capture.

The basis for obtaining the status of a single mother is the absence of a marriage record or the presence of a divorce certificate. However, a certificate is not enough. It is necessary to confirm the fact that the father really does not participate in the upbringing of the child. For this, the mother can apply to the court with a claim to deprive him of his parental rights, which will be decided by the court. You can also turn to the police in connection with the need to search for the father in the case of a claim for the collection of alimony. In this case, the relevant resolution of the investigator will be the document that confirms the fact of the father's non-participation in the upbringing of the child. Also, a certificate from a school, kindergarten stating that the father does not take part in the child's upbringing, or a similar document drawn up and signed by neighbors can be the basis. It should be added that the specified list of documents is not exhaustive, and there is no single established document sample that would certify the status of a single mother.

You can submit an application for receiving the service by registered letter to the local body of social protection of the population or other authorized persons.

If the certificate of a participant in hostilities has suffered significant damage or has been lost, a new certificate is issued upon the person's report (application). The report (statement) contains explanations regarding the circumstances of damage or loss of the certificate of a participant in hostilities. In case of loss or physical damage, new coupons and badges will not be issued. It is necessary to return to the TCC or a military unit.

The passport is exchanged in case of:

  • changes to the information entered in the passport (except for additional variable information);

  • obtaining the registration number of the taxpayer's registration card from the State Register of Individuals - Taxpayers (RNOKPP) or notification of refusal to accept the specified number (optional);

  • detecting an error in the information entered in the passport;

  • expiration of the passport;

  • unsuitability of the passport for further use (the passport/photo card is damaged (and/or its part is missing), which makes it impossible to visually identify the person, to read the surname, first name, patronymic, date and place of birth, who issued the passport , signature of an official and date of issue, damage to the perforated series and numbers that do not allow to establish passport details, corrections, including changes to the personal data of the person/names of the authority/stamp/seal, damage that blocks the possibility of reading, as well as

  • making changes to contactless electronic media information);

  • if the person has reached the age of 25 or 45 and has not applied in accordance with the procedure established by law no later than one month after reaching the relevant age for pasting new photo cards to the passport of the 1994 model;

  • the person has a 1994 model passport (optional).

To exchange a passport, the applicant submits the following documents:

  1. passport to be exchanged;

  2. documents confirming the circumstances in connection with which the passport is subject to exchange;

  3. certificate of registration of homeless persons (for homeless persons);

  4. a document certifying the identity of the legal representative/authorized person, and a document confirming the authority of the person as a legal representative/authorized person;

  5. documents confirming the payment of the administrative fee, or the original document on exemption from its payment. The original of the document on exemption from payment of the administrative fee is returned to the applicant, and a copy of it is attached to the application form, certified by an employee of the territorial body/territorial subdivision of the DMS, the authorized subject by marking "According to the original" and a signature indicating his position, surname, initials and date;

  6. documents confirming the information for entering additional variable information into the contactless electronic medium and into the passport (if such documents are available);

  7. one photo card measuring 10 x 15 centimeters for entering a digitized image of a person's face by scanning using the means of the Unified State Demographic Register - for issuing a passport to a person who cannot move independently due to a long-term health disorder, which is confirmed by a medical opinion of a relevant health care institution, issued in accordance with the established procedure, or to a person who is serving a sentence in a penal institution or in respect of whom a preventive measure in the form of detention has been chosen, or who is undergoing long-term inpatient treatment in a closed-type Ministry of Health institution (if necessary );

  8. a passport of a citizen of Ukraine for travel abroad, including an expired one (for a person who permanently resides abroad, or who permanently resided abroad and returned to Ukraine for permanent residence);

  9. application for removal from consular registration (for a person who has reached the age of 14, has permanently lived abroad and returned to Ukraine for permanent residence);

This issue is regulated by the regulations of the Resolution of the Cabinet of Ministers of Ukraine dated March 20, 2022 No. 332 (taking into account the changes introduced by Resolution No. 709) "Procedure for providing accommodation assistance to internally displaced persons".

According to this resolution, the opening of any FOP group is not an obstacle to receiving residence assistance for IDPs and cannot be a reason for their cancellation, and at the same time, this fact does not affect the loss of IDP status. Other factors affect the cancellation of payments: acquisition of real estate, vehicle, opening of deposit accounts, etc.

The inheritance includes all the rights and obligations that belonged to the testator at the time the inheritance was opened and did not cease as a result of his death, it can be both movable and immovable property, money, deposits, as well as debt obligations (Articles 1218, 1281 of the Civil Code of Ukraine).

Debts, including those under credit agreements, are considered part of the inheritance and pass to the heirs. It is impossible to accept only the rights and renounce the obligations connected with the inheritance, as well as to accept only part of the inheritance.
