You need to contact the guardianship authority at your place of residence and provide the following list of documents:
a statement that you want to provide permanent care for a certain person;
Your passport (original and copy);
passport of the person who will be cared for;
medical certificate about the caregiver's health;
certificate of registration of your place of residence;
documents regarding ownership of property or the right to use it;
a certificate from the medical commission (MSEK), which confirms that the person needs constant care due to his health condition.
If we are talking about a person with legal capacity who can be aware of his actions, but needs care due to his health condition, his mandatory consent is required. Therefore, when it comes to a pensioner, he must also write a statement to appoint a person who will provide him with constant care.
