List of documents for registration of candidates for adoptive parents:

  1. Copy of the identity document (3 copies).

  2. Copy of the registration number of the taxpayer's registration card (3 copies).

  3. A copy of the document indicating the unique number of the entry in the Unified State Demographic Register (if available) (3 copies).

  4. Certificate of salary for the last six months or information from the State Register of natural persons — taxpayers about the amounts of income paid and withheld taxes, or certificate of the submitted declaration of property status and income for the previous calendar year. (original and 2 copies).

  5. A copy of a marriage certificate or an extract from the State Register of civil status acts of citizens on marriage (for married persons) (3 copies).

  6. A report on the state of health of each applicant, issued by a state medical and preventive institution (according to Appendix 3), (original and 2 copies).

  7. An extract of the presence or absence of a criminal record for each applicant, issued by the territorial center for the provision of service services of the Ministry of Internal Affairs (the form of the extract is complete, in the grounds indicate: for adoption, establishment of guardianship/care, creation of a foster family or a family-type orphanage) ( original and 2 copies).

  8. A copy of the document confirming the right of ownership or use of the residential premises (3 copies).

  9. Notarized written consent of the second spouse to adopt a child (in case of adoption of a child by one of the spouses) (original and 2 copies).

  10. Certificate of completion of a training course on raising orphans and children deprived of parental care (for persons who are not relatives, guardians, custodians, adoptive parents, foster parents of a child who wishes to be adopted), (original and 1 copy ).

  11. Autobiographical certificate (in printed or electronic form).

According to Article 11 of the Law of Ukraine "On Copyright and Related Rights", the copyright to a work arises as a result of the fact of its creation. The creation and exercise of copyright does not require the registration of a work or any other special design of it, as well as the fulfillment of any other formalities.

If you are the author of a work (literary, musical, audiovisual, photographic, visual, etc.), it is important to certify your authorship in order to protect property and non-property rights to this work. For this purpose, information about the work is entered into the State Register of Certificates of Copyright Registration of the Work. For this you need:

  1. Apply to the State Enterprise "Ukrainian Institute of Intellectual Property" with the relevant application.

  2. Apply for the service in person or through a legal representative.

  3. Documents that must be provided to receive the service: application for registration of copyright to the work; a copy of the work in the prescribed material form; a document testifying to the fact and date of publication of the work (if the work has been published); a document on payment of a fee for preparation for registration and a document on payment of state duty for issuing a registration certificate or a copy of documents confirming the right to exemption from payment of a fee and state duty; a document on the payment of the state duty for the issuance of a certificate or a copy of the document confirming the right to exemption from the payment of the duty for the issuance of a certificate; a power of attorney drawn up in accordance with the established procedure, if the application is submitted by an authorized person on behalf of the author or his heir, or a copy of the power of attorney certified in accordance with the law; a document certifying the inheritance of the author's property right (if the application is submitted by the author's heir); instructions for using the database and description of the structure of the database (for registration of copyright to the database); instructions for using the program (for registering a copyright on a computer program); abstract, which must contain the name of the work, its location (address), architectural characteristics and parameters, time and place of creation (for registration of copyright on works of architecture, urban planning, garden and park and monumental art)

In case of copyright infringement, the author has the right:

  1. To demand recognition and renewal of one's rights, to prohibit actions that violate copyright or create a threat of their violation.

  2. Apply to the court with a claim for restoration of violated rights and termination of actions that violate copyright or create a threat of their violation.

  3. File claims for damages (material damage), including lost profits. Or about the collection of income received by the infringer as a result of his copyright infringement.

  4. File lawsuits for moral damages.

  5. To demand the cessation of preparatory actions for the violation of copyright, including the suspension of customs procedures, if there is a suspicion that counterfeit copies of works, phonograms, videograms may be passed into or out of the customs territory of Ukraine.

  6. To participate in the inspection of production premises, warehouses, technological processes and economic operations related to the production of copies of works, phonograms and videograms, in respect of which there are grounds for suspecting a violation or threat of violation of copyright.

  7. Demand publication in the mass media of information on the facts of copyright infringement and court decisions regarding these infringements.

  8. Demand from violators to provide information about third parties involved in the production and distribution of counterfeit copies of works, as well as means of circumventing technical protection measures, and about their distribution channels.

Methods of copyright protection:

Subjects of copyright have the right to apply to the court in accordance with the established procedure for the protection of copyright. If there are reasons, you can contact the National Police. The first and most effective measure in the process of protecting and restoring the violated rights of the author is to send a claim to the address of the person who committed such a violation. If signs of such a violation are detected on the Internet, then a substantiated claim, with an explanation of the arguments and a reference to the norms of the current legislation, must also be sent to the hosting provider and the owner of the domain name of the relevant resource.

  • Violation of the registration procedure in the state tax service bodies — a fine of 340 hryvnias (for self-employed persons)/1020 hryvnias (for legal entities); the amount of the fine is doubled for a repeated offense or if the offense is not remedied.

  • Violation of the deadline and procedure for submitting information on opening/closing bank accounts, electronic wallets — a fine of 680 hryvnias for each case; carrying out expenditure transactions on the taxpayer's account before receiving a notification from the state tax service about taking the account into account - a fine in the amount of 10% of the sum of all transactions carried out by a bank or other financial institution with such an account, but not less than 1700 hryvnias.

  • Violation of the procedure for submitting information about natural persons — taxpayers — a fine of 1,020 hryvnias; for repeated violation - 2040 hryvnias.

  • Improper preparation of documents (without specifying the registration number of the taxpayer's account card) — a fine of 340 hryvnias.

  • Non-submission or untimely submission of tax reporting; failure to comply with the requirements for making changes to tax reporting — a fine of 340 hryvnias; for a repeated violation - a fine of 1,020 hryvnias.

  • Violation of the terms of storage of certain documents established by law — a fine of 1,020 hryvnias, for repeated violations — 2,040 hryvnias.

  • Violation of the rules of application of the simplified taxation system by an individual — an entrepreneur — a fine of 50% of the single tax rate.

  • Alienation of property that is in a tax lien — a fine in the amount of the value of the alienated property.

  • Violation of the rules of payment (transfer) of a monetary obligation — in case of a delay of up to 30 calendar days, 5% of the repaid amount of the tax debt; with a delay of more than 30 days — 10%.
