The basis for accepting an application for the renewal of the act record of civil status is an extract from the State Register of acts of civil status of citizens on the absence of an act record of civil status.

Together with the application for the renewal of the lost legal record of civil status, the following are submitted:

1. an archival certificate issued by the state archive in the Autonomous Republic of Crimea, the region, the cities of Kyiv and Sevastopol, if the metric books or books of state registration of acts of civil status are transferred to a state archival institution for safekeeping;

2. documents (extracts from them) that confirm the data necessary for renewal of the act record of civil status;

3. certificates of state registration of acts of civil status (birth, death, marriage, dissolution of marriage, etc.);

4. other documents necessary for considering the application and solving the issue on its merits.

On the basis of the updated act record of civil status, the applicant is re-issued or sent to the department of state registration of civil status acts at his place of residence, a certificate of state registration of the act of civil status for delivery. The stamps "Record renewed" and "Repeat" are affixed to the certificate.

In accordance with Part 2 of Art. 23 of the Law of Ukraine "On State Registration of Civil Status Acts", an application for renewal of the civil status act record is submitted to the relevant body of state registration of civil status acts at the applicant's place of residence.
