Documents for obtaining IDP status in Ukraine if you lose your passport

First, you need to restore your lost passport. Contact the Citizen Service Center "Passport Service". To issue a passport of a citizen of Ukraine in the form of an ID card, instead of a lost or stolen one, you must submit:

1. statement about the loss of a passport;

2. extract from the ERDR (in case of passport theft);

3. a document confirming payment of the administrative fee, or the original document on exemption from its payment;

4. a certificate from the registration authority of the established form about the place of residence (if available).

After you receive a passport of a citizen of Ukraine, you can contact the Central Social Assistance Center or the Department of Labor and Social Protection of the Population to register the status of an internally displaced person.
