If the foreign passport has been lost or expired, a new one must be made. The following documents are required to renew a foreign passport:

* application – questionnaire;

* application for loss of foreign passport;

* passport of a citizen of Ukraine or birth certificate (for persons under the age of 14);

* a receipt for the payment of the administrative fee;

* extract from the Unified Register of Pretrial Investigations (in case of theft of a foreign passport).
