To obtain a certificate of residence without registration of residence, the applicant submits:

• application in the established form;

• an identity document (a passport of a citizen of Ukraine; a child's birth certificate (for minors); for foreigners and persons without Ukrainian citizenship, an identity document confirming the applicant's special status);

• a document confirming the place of registration/deregistration of the place of residence of the person in whose name the certificate is issued (excerpt from the register of the territorial community;

• a document confirming the right to represent a person's interests in accordance with the procedure established by law (in the case of an application by a representative);

• a document confirming ownership of a residential building/apartment (if necessary);

• certificate of residence of a person without registration of place of residence.
