What design algorithm isMalyatko?

You can order the eMalyatko offline service with the help of an administrator in the maternity hospital (before discharge), the National Medical Center or the Department of the National Center for Social and Health Care.

What documents are required:

• original passports of parents or digital passports in the Diya application;

• medical report number or original medical birth certificate (form 103/о, issued in the maternity hospital);

• tax numbers of both parents — originals are not required, it is enough to verbally name the number, have scanned copies or a photo;

• information about the date, city and authority that concluded the marriage (if the parents are married), the originals are not required, it is enough to dictate the information verbally, have scanned copies or photos;

• UZR of one of the parents for assignment of UZR to the child (if one of the parents has UZR);

• bank account number in IBAN format for crediting child support.

You may also need:

• written consent of one of the parents regarding assigning a surname to the child (if the parents have different surnames and one of them submits the application);

• written consent of the second parent regarding the registration of a child born with one of them (if the parents are registered at different addresses and one of them submits the application).
