Women who are registered with the employment center as unemployed are entitled to state assistance in connection with pregnancy and childbirth.
To assign assistance to such a woman, she must contact the territorial body of the Pension Fund of Ukraine with the following documents:
passport (or other identity document);
the registration number of the taxpayer's registration card (if available);
an application indicating, in particular, the chosen payment method (bank details or post office details). The application form is approved by the order of the Ministry of Social Policy of Ukraine dated January 9, 2023 No. 3 “On Approval of the Application Form for the Assignment of All Types of Social Assistance and Compensation”, registered with the Ministry of Justice of Ukraine on January 23, 2023 under No. 145/39201;
information about the record number (16-digit code) in the Register of Medical Opinions in the electronic health care system about the formed medical opinion on temporary incapacity for work of the category "Pregnancy and childbirth", which can be obtained from a doctor or in your personal account on the web portal of electronic services of the Pension Fund of Ukraine;
a certificate from the employment center stating that the woman is registered with the employment center as unemployed;
ID card of a person who suffered as a result of the Chernobyl disaster (if available).
Documents can be submitted in one of the following ways:
when personally contacting any service center of the Pension Fund of Ukraine;
through the executive body of the village, settlement, city, district council in the city;
through the administrative services center;
by mail – to the address of the territorial department of the Pension Fund of Ukraine in the region, Kyiv;
in electronic form – through the web portal of electronic services, the mobile application of the Pension Fund of Ukraine or the Unified State Web Portal of Electronic Services (Portal Diya) or by means of the Social Web Portal of Electronic Services with a qualified electronic signature (if technically possible).
