Am I, as a citizen of Ukraine, obliged to undergo a medical examination at a state hospital if I have received a referral from my family doctor, and what could be the consequences of refusing?

Dad, 81 years old, is in the hospital after having his leg amputated. How should I issue him a disability group? I was told that I should get a referral from the family doctor and bypass all the doctors, and he is bedridden.

The man was discharged from the hospital, but his health did not improve. Tell me who gives the referral to the VLK and how to achieve this correctly? Thank you!
