How to change your name?

In order to change the name, you need to contact the department of state registration of civil status with the appropriate application.

This application is submitted in writing. The department considers it three months from the date of submission. For a good reason, this term can be extended, but not more than three months.

The application for name change must be submitted:

• passport;

• Birth Certificate;

• certificate of marriage or its dissolution;

• birth certificate of children, if they are minors or minors;

• a certificate of change of name of the applicant, father or mother, if it was previously changed;

• own photo card and a receipt for the payment of state duty in the amount of 5 hryvnias and 10 kopecks.
