Even if the divorce took place through the court, the department of state registration of acts of civil status does not have the right to refuse you the service of changing the surname.

Contact DRATSS again with:

  • a written statement about the return of the maiden name after the divorce;

  • documents confirming the intention to change the surname after the divorce: a court decision indicating the desire to restore the maiden name after the divorce or a divorce certificate;

  • photo card;

  • a receipt for the payment of state duty (except in cases of exemption of the applicant from payment of state duty) - in the amount of 0.3 of the non-taxable minimum income of citizens (5 hryvnias 10 kopecks) and 3 non-taxable minimum income of citizens - upon repeated change of surname (51 hryvnias).
