You have a driver's license in the Diya application. You can order a new driver's license at the service center of the Ministry of Internal Affairs (only in Ukraine):

1. Enter the driver's electronic account using an electronic signature

2. In the electronic services section, select "Driver's license renewal and exchange".

3. Fill out the application and attach electronic copies of documents confirming the change of personal data, a police certificate and a valid medical certificate form 083/о (optional).

4. Apply an electronic signature.

5. Make payment using the electronic payment system.

6. Wait for a notification in case of successful verification of the application and documents in state registers and databases.

7. Get a driver's license at the territorial service center of the Ministry of Internal Affairs indicated in your electronic application.

Therefore, you can restore your lost driver's license online, but you can get it only on the territory of Ukraine. That is, the way out of the situation is either to go to Ukraine to get a driver's license or to get a Polish driver's license.
