How to get an extract from the E-hospital?

To receive an E-hospital extract, you need to perform the following actions:

1. create your own account on the web portal of the Pension Fund of Ukraine https://portal.pfu.gov.ua. To do this, you need to authorize using an electronic digital signature or Bank-ID.

2. After authorization, go to the "Communications with PFU" section.

3. Click the "Request for paper documents" item.

4. Next, select the type of request "Extract from the ERLN".

5. Enter your data and click "Send to PFU"

On the same portal, in the "Request" section, you can view information about the status of your request, as well as download it in PDF format.
