Issuance of a passport of a citizen of Ukraine with a contactless electronic medium in the form of an ID card takes place:

  • in exchange for a 1994 model passport (attainment of 25/45 years of age);

  • in exchange for a 1994 model passport (optional);

  • in case of lost/stolen passport:

  • in case of expiry of the passport;

  • in case of information change;

  • in case of detecting an error in the information;

  • if the passport is unfit for further use;

  • in case of assignment of RNOCPP or notification of refusal to accept it.

List of required documents:

  1. Passport of a citizen of Ukraine;

  2. Documents confirming the circumstances in connection with which the passport is subject to exchange (marriage/divorce certificate/name change/court decision on divorce) (together with a certified translation, if necessary);

  3. RNOCPP or notice of refusal to accept it (if available);

  4. Certificate of residence registration (if available).

  5. Extract from the Unified Register of Pretrial Investigations (in case of passport theft).

*If the person received a passport of a citizen of Ukraine of the 1994 model in the territory that is currently not under the control of Ukraine (individual areas of Donetsk, Luhansk Oblasts and the AR of Crimea) - additionally submit all available documents, including documents containing photographs of the person, which make it possible to identify such a person

When issuing a passport, a verification procedure is carried out. The presence of the person in whose name the document was issued is mandatory. In the event that a citizen does not receive a passport within three months from the date of its receipt at the "Passport Service" center, the completed document is returned to the territorial division of the Ministry of Internal Affairs for further storage. If the applicant does not receive a passport within two years from the date of its issuance, it will be destroyed in the territorial division of the State Immigration Service. At the written request of the applicant, the term of keeping the passport can be extended up to three years from the date of its issuance.
