Certificate of the medical and social expert commission on establishing the percentage of incapacity for work and the decision of the relevant military medical institution regarding the recognition of the injury.

If you are a private or non-commissioned officer, you must present a military ID and its certified copy, as well as a certificate of military service. For officers - a certificate or military card of a reserve officer and its copy and extract from the record.

Documents of military medical examination: certificate of illness, certificate of military medical commission, originals and their certified copies.
