Bureaucratic error in military registration

I read that a man ended up in the database of wanted conscripts due to a bureaucratic error, although there were probably no grounds for it. I am concerned that this can happen to anyone. If I am suddenly groundlessly added to such a database, how should I act in this situation? What steps need to be taken to prove the error and remove myself from this list, and can those responsible be held accountable?
