In order to restore the passport of a citizen of Ukraine, submit an application for the loss of a passport document when applying in person to one of three institutions: to the Department of Internal Affairs of Ukraine, to the Center for the Provision of Administrative Services or to the "Passport Service" citizen service center.

To issue a passport of a citizen of Ukraine in the form of an ID card, instead of a lost one, submit the following documents:

• application for loss of passport;

• a document confirming the payment of the administrative fee, or the original document on exemption from its payment;

• certificate of the registration body of the established model about the place of residence (if available).
