How to properly apply for a new UBD certificate in accordance with the updated rules of the Ministry of Defence of Ukraine? What documents need to be provided and where to apply in case of loss or theft of the previous certificate?

There are two ways to obtain the status of a combat participant. The first is automatically through the Unified State Register of War Veterans, where authorized persons of military units are required to enter the relevant information within five days from the start of a combat mission. The second is by personally submitting documents to one of the 21 commissions of the Ministry of Defense that handle the review of such materials.

For self-submission, you must provide:

  • an application in the established form
  • two 3×4 photos on matte paper
  • documents confirming participation in combat operations.

The application is sent by mail to the commission of the military administration body to which the military unit is subordinated. If the applicant does not have the original necessary documents, the commission can request them independently through interdepartmental cooperation.

What to do if the UBD certificate is lost?

  • Active military personnel submit a report to the unit commander.
  • Those discharged from military service submit an application to the TCC and SP at their place of registration.
  • Document restoration is possible only after submitting a statement to the National Police about the loss of the certificate and publishing an announcement of its invalidity in print media.
