UBD status: questions about receiving

If a serviceman performed combat missions, but due to certain circumstances, the authorized person of the military unit did not enter the relevant information into the Unified State Register of War Veterans within five days, can he independently apply to the commission of the military management body to obtain the status of a UBD?
