Persons who are citizens of Ukraine will need a minimum package of documents:

  • a statement with a request to issue a relevant certificate, written by hand;

  • copy of passport and individual tax number;

  • the power of attorney is certified by a notary, if the person does not apply in person.

Persons with citizenship of another country will need much more documents:

  • an application with a request to issue a certificate confirming the status of a resident of the country;

  • a copy of the passport with a notarized translation;

  • individual tax number;

  • work permit from the employer;

  • migration card or visa (copy);

  • a power of attorney certified by a notary if the interests will be represented by a third party;

  • a document confirming the fact of living in the territory of Ukraine or owning real estate in the territory of Ukraine.
