Misuse of humanitarian aid

Please tell me if the humanitarian aid was recorded in the journal in an arbitrary form when it was issued: only the surname, age of the child, number and preferential category were written without the signature of the recipient (baby diapers were issued for children of IDPs, those with many children, the poor, the military), this is a non-targeted use of humanitarian aid help? Do parents who receive, for example, diapers, need to provide documents to receive humanitarian aid, and if so, which ones?
