referral for a medical examination of the VLK;

medical book;

identity card (military card or officer's card);

medical characteristics (not a mandatory document in wartime);

job description (not a mandatory document in wartime);

the original of the certificate on the circumstances of the injury from the accident;

a certified copy of the accident certificate (accident certificate is not issued in case of injury while participating in hostilities, but only if the injury was received during service, for example, a broken arm);

UBD certificate (if available);

excerpts from personal files and other reference materials for combatants.
