Good day.

Registration of care for a person with a disability with a mental disorder who needs constant care is provided by an able-bodied person who is registered or permanently lives with such a person.

In order to receive cash assistance for care, it is necessary to submit an application and documents, such as certificates of family income, documents certifying the identity and family ties, and the opinion of the medical commission on the need for permanent external care.

If the person lives in a different place than the one registered, it is necessary to confirm the fact of joint residence and care according to the act of examination by specialists.

The following documents are submitted for the appointment of cash assistance for care by the person who applies for its appointment:

  • statement;

  • an identity document;

  • declaration of income and property status;

  • the opinion of the medical commission of the medical institution regarding the need for permanent outside care of a person with a disability of group I or II due to a mental disorder, regardless of whether mental disorders are the main cause of disability;

  • a copy of the certificate of inspection by the medical and social expert commission, which was issued to a person with a disability of group I or II due to a mental disorder, for whom care is provided.

The application with the necessary documents is accepted by authorized officials of the executive body of the village, settlement, city council of the relevant territorial community and officials of the administrative services center.
