Is a letter from the State Social Security Administration with a wet seal stating that the original medical report on the child's disability is available in the case files sufficient for the CCC, instead of a certified copy of the report, as required by law, or can this become a formal basis for refusal?

To confirm a child's disability, you must provide:

  • a medical certificate for a child with a disability under the age of 18 in a form approved by the Ministry of Health, or an individual rehabilitation program for a child with a disability, issued by the medical advisory commission of a medical and preventive institution; to confirm family relationships with parents (for biological parents)

  • a birth certificate of a child indicating the paternity of the conscript; to confirm the powers of a guardian, trustee - a decision of the district, district in the cities of Kyiv and Sevastopol state administration, an executive body of a city, district in the city (in case of formation) council, village, settlement council of a united territorial community or a court decision on the establishment of guardianship, custody and a birth certificate of a child; for adoptive parents - a court decision on adoption and a birth certificate of a child indicating the paternity of the conscript.
