How to get a military registration document?

In order to receive a document, you need to contact the administrator at TsNAP, provide up-to-date information about yourself and confirm it. After entering the information into the electronic system, it will automatically be uploaded to the "Amulet".

To be included in the military register, all conscripts must personally come to the TCC and SP at their place of residence and submit an application for their inclusion in the military register.

Documents that must be provided to receive the service:

1. Application

2. Passport of a citizen of Ukraine

3. Military ID or temporary ID of a conscript

4. For persons released to the reserve: a service record card, a prescription from a military unit, and for those released due to health or family circumstances, a certificate of illness and other documents that confirm the legality of early release

5. Decisions of military medical commissions on fitness for military service in peacetime or wartime (if available).
