How to get a certificate of a large family?

Structural subdivisions of district, district in mm. In Kyiv and Sevastopol, state administrations, executive bodies of city (cities of regional importance), district councils in cities (in case of formation), village, settlement, city councils of united territorial communities, or the center for the provision of administrative services issue a certificate of a large family, a certificate of a child with of a large family, which confirms their status.

The applicant can apply for the service in person or through a legal representative, by sending documents by mail (registered mail).

Documents that must be provided to receive the service:

1. Certificate of family composition

2. Father's or mother's passport

3. Birth certificate of children

4. One photo card measuring 30 × 40 millimeters

5. In case of extension of the term of validity, if the child is studying full-time in an institution of general secondary, professional (vocational-technical), professional pre-higher, higher education, - a certificate confirming enrollment in the educational institution or transferring to the next course of study within of the relevant training program (information is confirmed by the applicant every semester, before graduation from the educational institution, but no longer than before the person reaches the age of 23)

In case of registration of remarriage and living with children from a previous marriage - documents confirming the fact that the child is raised by one of the parents (a copy of the court decision indicating the place of residence of the child with one of the parents after the divorce; a copy of the court decision on deprivation of the father or mother parental rights, recognition of the father or mother as incapacitated (incapacitated), missing (absent);
