The applicant can apply for the service in person, by sending documents by mail (registered mail).
Documents that must be provided to receive the service:
Application for the assignment of state social assistance to low-income families.
Declaration of income and property status of persons who have applied for all types of social assistance.
Certificate of the established form on the direct participation of a person in an anti-terrorist operation, in the implementation of measures to ensure national security and defense, repel and contain the armed aggression of the Russian Federation in the Donetsk and Luhansk regions (if available).
A certificate of income in the case of the declaration of income, the information about which is not available in the DPS, the Pension Fund of Ukraine, social insurance funds, etc., and in accordance with the law, cannot be obtained at the relevant request of the social protection body.
Applications with the necessary documents are accepted from citizens by the social protection bodies of the district state administrations only if they are sent by mail or in electronic form (through the official website of the Ministry of Social Policy, the information systems of executive authorities and local self-government bodies integrated with it, or the Unified State Web Portal of Electronic services).
Applications with the necessary documents are also accepted by authorized officials of the executive body of the village, settlement, city council of the relevant territorial community and officials of the center for the provision of administrative services, who transfer them within three working days from the date of their receipt to the relevant public social protection body.
Applications with the necessary documents (after the creation of an electronic case) in paper form are submitted to the relevant body of social protection of the population at least once every two weeks.
