As a general rule, an application for the appointment of a pension is submitted to the bodies of the Pension Fund in the district, city, district within the city, as well as in the city and district at the place of residence (registration).

In addition to the application, the following documents must be submitted:

• an identity document;

• a document confirming the registration number of the taxpayer's registration card (except for persons who, due to their religious beliefs, refuse to accept the registration number of the taxpayer's registration card and have notified the relevant supervisory body about it and have a mark in their passport), or a certificate of mandatory state Social Insurance;

• seniority documents specified in the Procedure for confirmation of existing seniority for awarding pensions in the absence of an employment book or relevant entries in it;

• a certificate of the person's salary for the period of insurance experience until July 1, 2000, and starting from July 1, 2000, individual information about the insured person is provided by the personalized accounting department;

• documents on the person's place of residence (registration);

• documents certifying the special status of a person: a certificate of a participant in the liquidation of the consequences of the accident at the Chornobyl NPP or a certificate of the period(s) of participation in the liquidation of the consequences of the accident at the Chernobyl NPP or a certificate of the military unit in which the person participated in the liquidation of the consequences of the accident at the Chornobyl NPP , or a certificate from an archival institution, or other primary documents, which indicate the period of work, the settlement or the object where the person performed work to eliminate the consequences of the accident at the Chornobyl NPP;

• a certificate of a victim of the Chernobyl disaster and a certificate about the period(s) of residence (work) in the territories of radioactive contamination, issued by local self-government bodies (enterprises, institutions, organizations);

• photo card (for issuing a certificate).

Necessary documents:

• Completed application.

• Document proving the identity of the applicant.

• Tax number or series and passport number (for individuals who, due to their religious beliefs, refused to have a tax number and have a note about this in their passport).

• Certificate of residence or a copy of the passport with data on the residence of the person for whom the assistance is issued.
