Assistance in connection with pregnancy and childbirth (decree) at the FOP is issued on the basis of a sick letter.
To apply for assistance, you need:
get a sick leave. It is discharged at the place of observation of the pregnant woman. Since 2021, the system of electronic sick leave has been operational, now you can not receive a sick leave in paper form;
open a special account for the payment of maternity benefits. Such an account can be opened in any bank. An ordinary account of an entrepreneur will not work;
submit a single tax reference declaration with appendix 1 for the current period, which indicates the accruals for the EUV for this period (the declaration and supplement indicate the period from the beginning of the year or the date of registration of the FOP, if the registration was this year, and up to the month of sick leave registration (including ));
go to the cabinet on the Pension Fund portal. Select the category "Application-calculation of payment for sick leave". Create a statement-calculation and attach (in PDF format): Statement of the FOP from the EDR and Extract from the EP, receipts for the payment of the ESR, a certificate of no debt from the ESR, reporting for the previous year;
fill in the calculation, sign and send. Based on the results of consideration of the calculation, an answer will come whether it is accepted or not.
