According to clause 267.4.1 clause 267.4 of article 267 of the Code of Civil Procedure, a business entity submits an application for the purchase of a trade patent to the supervisory body at the place of payment of the fee, which must contain the following information:

  1. name of the business entity, code according to EDRPOU (for a legal entity) and surname, first name, patronymic of the business entity, registration number of the taxpayer's registration card (for an individual);

  2. the legal address (location) of the business entity, and in case the patent is purchased for a separate subdivision, the location of such separate subdivision according to the document certifying the right of ownership (lease);

  3. type of business activity for which a trade patent will be purchased;

  4. type of trade patent;

  5. name of the document on full or partial payment of the fee;

  6. name and actual address (location) of the point of sale of goods, point of provision of paid services, point of exchange of foreign currency, gambling place, designation "outbound trade";

  7. name, date, number of the document certifying the right of ownership (lease);

  8. the period for which the trade patent will be purchased.

The basis for acquiring a trade patent is an application made in accordance with these requirements. The establishment of any additional conditions for the purchase of a trade patent is not allowed.

According to clause 267.4.2 clause 267.4 of article 267 of the Code of Civil Procedure, the information provided in the application submitted by the business entity shall be checked against the originals or notarized copies of the documents on the basis of which the application was filled out.

In case of inconsistency of the information provided in the application submitted by the business entity with the documents on the basis of which such application was filled out, or failure to enter all the necessary information in the application, the controlling body has the right to refuse to issue a trade patent to the business entity.
