How to get an extract from the voter register?

To obtain a certificate of inclusion in the state voter register in accordance with Article 21 of the Law of Ukraine "On the State Voter Register", it is necessary to personally apply to the body maintaining the register at your place of registration (voting address).

Documents that must be provided to receive the service:

1. Voter's statement

2. A document certifying identity and confirming Ukrainian citizenship - a passport of a citizen of Ukraine in the form of a passport book or card or a temporary certificate of a citizen of Ukraine (if the person recently acquired Ukrainian citizenship)

3. A document certifying the representative's identity and a document confirming the person's authority as a representative

4. A document certifying the voter’s inability to move independently or its copy (if there are grounds)
