Upon reaching the age of 16, you have the right to change your surname and first name at your own discretion. If you want to change your surname, you need to submit an application for its change to the state registration department of civil status acts at your place of residence. In addition, the following documents are attached to the application:
birth certificate of the applicant;
marriage certificate (if the applicant is married);
divorce certificate (if the marriage has been dissolved);
birth certificates of children (if the applicant has minor or minor children);
certificate of change of surname of the applicant, father or mother, if it was previously changed;
photo card of the applicant;
receipt for payment of state duty.
After you receive a certificate of name change, with it and a passport, you need to contact the migration service: if you have a passport-book - at the place of your registration, if you have an ID card - at any department of the service.
The procedure for issuing a passport of a citizen of Ukraine with a new surname does not differ from the procedure for issuing a normal passport.
After receiving a new passport, you can exchange other documents that contain a person's personal data.
